Dental Appointments
Appointments may be made by phone or by visiting the practice. If you have not yet made your first appointment, you can call our offices at 503-224-3853 and one of our team members will be glad to make an appointment for you.
Our office hours are Tuesday - Friday, 8:00 am-5:00 pm. Appointments outside normal working hours may be made by special arrangement.
Initial Consultation
Your initial consultation appointment will begin with a discussion regarding any dental concerns you may have. All special investigations, such as necessary radiographs, are obtained and a thorough examination is performed. The results of the findings will be discussed with you, and if any treatment is required, all possible options will be considered.
A written treatment plan with fee estimates will always be provided prior to any therapy being performed. This ensures you remain satisfied and aware of all recommended treatment.
Financial Policy
Nobody likes to be surprised when it comes to costs. Our policy is to inform you what your investment will be prior to any treatment and define financial arrangements with you before we begin. After your initial examination, we will review with you our recommended treatment plan and its projected cost.
We are proud to be a part of a team whose primary mission is to deliver the finest and most comprehensive Periodontal and Implant treatment available. To assist you with your healthcare investment, we are providing the following payment guidelines below.
Payment is due in full on the date of service.
Pre-Payment Plan
Our pre-payment plan is much like a Lay-Away plan. We accept whatever payments you feel comfortable with and begin treatment as soon as it is fully funded.
90 Days Same As Cash
This payment plan requires 1/3rd of the total due on the date of service, then 1/3rd the next month and 1/3rd the following month, all with no interest.
6 Month Payment Option
This payment option begins with a minimum of 50% down, then a payment for each of the next six months. No interest for 90 days, then a 1.5% finance charge per month thereafter, on the outstanding balance.
Financing
We have partnered with Care Credit and Lending Club financing options. Both of these options require pre-approval and provide up to 12 months of interest-free financing options OR up to 48 months low-interest financing.
Credit Cards Accepted
We accept Discover, Master Card and Visa.
Returned Checks
Dr. Haghighat and Portland Perio Implant Center reserve the right to charge $25 for returned checks, $5 billing fee in addition to 1.5% interest on balances of 90 days and any fees (including legal fees) incurred if an account is transferred for collection.
Patients With Dental Insurance
We welcome your dental insurance if it is an “open option”, and we will work with you to obtain your full benefit for our services. Please understand that you, through your employer, have a contract with that insurance carrier, and Kamran Haghighat, B.D.S., M.S., P.C. is not a party to that agreement. As such, it is not possible to predict exactly what each policy pays, although we always strive to be accurate. Because of this, it is important that you recognize that you are ultimately responsible for any balance. Patients who carry insurance should remember that services are rendered and charged to the patient, not the insurance company. We will be happy to file the insurance claim for you, but this is not a guarantee of payment by your insurance company. On the day of treatment, patients with insurance are required to pay their estimated portion of the fee.
Cancellation Policy
In an effort to keep costs down we respectfully ask that you give 72 hours or more notice for consultation appointment cancellations. This is to ensure we are allowed adequate time to contact patients who are either in urgent need of treatment, or are eager to continue with ongoing treatment. Your appointment time has been scheduled exclusively for you and a late cancellation or rescheduling will result in a $95 charge. Late cancellation of a surgical appointment (less than 7 working days notice) will result in a 5% charge. This policy allows us to offer our best services as efficiently as possible, to all patients at all times.
