The Initial Presentation
This patient presented with a severely compromised dentition resulting from advanced periodontal disease, extensive caries, multiple failing restorations, and significant bone loss. Clinical examination revealed generalized inflammation, heavy calculus accumulation, gingival recession, mobility of several teeth, and multiple hopeless teeth with poor restorative prognosis. The deterioration had progressed to the point where both function and esthetics were significantly compromised.

Using three-dimensional CBCT imaging integrated with digital intraoral scans, each implant was virtually positioned before surgery. This planning process allowed us to optimize implant angulation, restorative space, emergence profile, and long-term biomechanics.
Guided surgery accurately translated the digital plan to the patient, enabling precise implant placement while minimizing surgical trauma.
At Portland Perio Implant Center, much of this digital workflow— including treatment planning, CAD design, and 3D printing—is performed in-house. This level of control enables seamless communication between the surgical and restorative teams, improving efficiency and accuracy throughout treatment.

Following the removal of the hopeless teeth, four strategically positioned implants were placed to maximize available bone and provide immediate stability for a fixed provisional restoration.
Whenever clinical conditions allow, we provide patients with fixed teeth on the day of surgery. This “teeth-in-a-day” approach allows patients to leave our office with a functional smile rather than a removable denture.

During the healing phase, the provisional prosthesis served as a blueprint, allowing refinements in esthetics, phonetics, and function before fabrication of the definitive restoration.

After complete osseointegration, definitive implant positions were captured using advanced digital photogrammetry. This technology provides an exceptionally accurate digital record of implant locations, minimizing distortion commonly associated with conventional impressions and contributing to a highly passive, precise prosthetic fit.
The final restoration consisted of a high-strength monolithic zirconia prosthesis designed to provide excellent esthetics, long-term durability, and optimal function. Careful attention was given to occlusion, hygiene access, and natural emergence profiles to maximize both longevity and patient comfort.

Some patients are excellent candidates for All On 4, while others may benefit from six implants supporting an FP1 restoration that preserves more natural bone and soft tissue architecture. In some situations, preserving the natural dentition remains the best option.
